Options for the FBI Fingerprint Background check requirement:

All volunteers are required to provide the result of an FBI Fingerprint Background check or, if they have been a Pennsylvania resident for at least ten consecutive years, a disclosure statement affirming as much.  If you have been a Pennsylvania resident for at least ten consecutive years, please use the Residency Affidavit Disclosure Application form.  If you do not meet the residency requirement, you can submit a copy of an FBI Fingerprint Check result as required by an employer or other volunteer position.  If you don't already have the result of a check for another purpose, you will need to get go through the process of getting fingerprinted to get the results of a new background check.

The residency affidavit or FBI fingerprint background check must be current within the last five years.  The church will let you know when it is time to submit a new residency affidavit or get a new background check.
